1. A simple trick would enhance your PDF a lot. Simply read your
   doc file into OpenOffice.org and from there export it to PDF.
   Why? Because all your table of contents would be linked to the
   write pages and all the URL you wrote would automagically
   fire your web-browser to the relevant web page.

2. Amir Aharoni replied that a .doc is prefered to PDF. I beg to
   differ (strongly). Currently PDF is the *only* format that assure
   you the final look of the book (page breaks, RTL, justification,
   special characters, etc.) Therefore, it's the only acceptable
   format for technical publishing.

3. Since you decided to license your document as GFDL you need to
   specify what are the invariant parts (if that's the whole book
   and not only your credits etc, than you need to specify it).
